Description:
To provide a hot, nutritious meal to participants striving to remain independent in their homes. In addition to meal provision, services include nutrition screening, education, nutrition assessment and counseling as appropriate. Home delivered meals also represent an essential service for many caregivers, by helping them to maintain their own health and well-being. In an effort to meet rural participant needs multiple meals are delivered at one time each week and wellness calls are placed daily as a means of providing a safety check.

Eligibility
Individuals who are age 60+, the spouse of a 60+ individual regardless of age. May be available to individuals who are under age 60 with disabilities if they reside with a homebound older individual. Call for specific eligibility requirements.

Call the office or complete and submit a referral form on the website to schedule a home visit.
