Eligibility
Individual must self-certify they have a hearing loss that necessitates the use of captions to use the phone effectively. Must provide registration information including: full name, date of birth, last 4 digits of SSN (or acceptable substitute). All documents are obtained during the registration process. IP CTS is a program based on the ADA requirement for functionally equivalent access.

Complete application on the CaptionCall website, call CaptionCall to request a phone, or download the app.
