0-18 years of age with a medical card or no insurance; Infant through adult with private insurance.
Fees
Most insurance accepted, private pay, Medicare.
Intake Procedure
Walk-In Tues 8:30 am - 11 am and 1:00 pm - 4:00 pm; Fri 1:00 pm - 4:00 pm (2nd Tues of the month 1 - 6 pm).
