Description

Children who have medical needs are sometimes not insured comprehensively to provide coverage for all their medical treatments. There are few places for families who have gaps in their commercial health insurance coverage to turn to for funding medically necessary services for their children. Children may go without necessary treatment, or, they receive the care and families assume a large amount of debt. The Foundation understands these needs and is willing to help fill this void. Eligibility

Any child, 16 years old or younger living in any UnitedHealthcare region of the United States in need of financial assistance for health care services. Must have a social security number and meet income guidelines. The applicant must be covered by commercial health insurance policy and limits for the requested service are either exceeded, or no coverage is available and/or the copayments are a serious financial burden on the family. Health insurance sponsored by the Federal government (e.g. Medicaid, etc.) does not qualify as a commercial health insurance company. More eligibility information found on our website.

Intake Procedure

Each applicant is required to submit a grant application. Each application will be evaluated based on criteria approved by the National Board of Directors. Once an application passes this evaluation process, it will be presented to the Regional Board of Directors for review.
