Eligibility

Eligibility services are available for disabled individuals applying for Medicaid that are 16 years of age or older. Elderly services are provided to individuals 60 years of age or older that qualify based on their health score.

Fees

Some services are provided on a sliding fee scale. These fees are paid to the provider of services.

Intake Procedure

The individual in need of services should contact the agency intake person. This person must be willing to share income and asset information. Many programs require a co-pay amount to receive services. Some services are offered at little or no expense, however individuals will be asked to make a donation for those services.

Allen, Bourbon, Cherokee, Crawford, Labette, Montgomery, Neosho, Wilson, and Woodson counties.
