Limited phone hours of 1-3 pm Tues - Fri.
Eligibility
Children ages 7-18 years from families that meet income guidelines. Must have a moderate to severe malocclusion (crooked teeth/jaws), good oral hygiene, and not already wearing braces.
Fees
$30 application fee and $650 required investment if approved for treatment through Smiles Change Lives.
Intake Procedure
Fill out application available online at www.smileschangelives.org and review checklist for all supporting documentation needed and mail as directed. Email is preferred method of contact.
