Fees
The Clinic is fee for service clinic. We have a sliding scale as well as scholarships for those who need assistance.
Intake Procedure
To request services from the Schiefelbusch Clinic complete the Intake Questionnaire form and fax or mail it to the Clinic. Someone from the Clinic will contact you to discuss the services you are requesting. Please allow 48-72 hours after receipt of the form for this contact to be made. If you are not contacted, please contact the Clinic at 785-864-4690.
