Application, child/children's birth certificate, immunization records, child's CDIB, proof of residency, one month of income from the applicant or applicants.
Eligibility
Member of a federally recognized tribe. Currently employed or seeking employment. Currently enrolled in school and/or an education program. Currently enrolled in job training.
