Description:
A voluntary and intensive home visitation program designed to help new parents. Parents who qualify may enroll in the program prenatally or within the first three months after birth. Services are voluntary and are available until the child is 3 years old. Home visitors provide support to families through weekly home visitation which includes child development information, parent-child interaction activities, and support with setting and achieving goals. Home visitors provide referrals to needed resources for families. Limited assistance with transportation to needed medical and WIC appointments is available for enrolled families.
Hours:
8 am - 5 pm Mon - Fri. Home Visits may be conducted after 5 pm depending on the family's schedule.

Eligibility
Families qualify based on multiple risk factors, which could include history of childhood abuse, domestic violence, mental illness or drug use. Other risk factors include low income, lack of stable housing, unemployment, lack of education, single parents, and more. Eligibility is determined during an in person meeting with the Family Engagement Coordinator.

None.
Intake Procedure
Families who are interested in the program will be asked to schedule an initial home visit to meet with the Family Engagement Coordinator to learn more about the program and determine eligibility.
