Federal funding through HRSA, Harvey County United Way, KDHE Primary-care program, private foundations, local city and county government, churches, individual donations, and patient fees.

Description

Provide primary medical, dental and targeted behavioral health services to the entire community regardless of ability to pay. Not all services are provided at each location. Ancillary services include laboratory, diagnostic imaging (i.e., X-ray, CT scan, and DEXA scan), pharmacy, medication assistance program, certified federal insurance marketplace navigators, and transportation. In addition to the main clinic there are two satellite locations: Halstead (primary medical) and a second site in Newton (Dental).

Main Clinic at 720 Medical Center Dr: 8 am - 5 pm Mon, Wed, Fri; 8 am - 7 pm Tues & Thurs. Dental at 215 S. Pine: 8 am - 5 pm Mon - Thurs and 8 am - 12 pm Fri. Halstead Clinic at 126 N. Main: 8 am - 5 pm Mon - Fri.

Eligibility

All are welcome.

Fees

Sliding scale available for those between 0% and 200% of Federal Poverty Guidelines (proof required). Accepts Medicaid, Medicare, and most major commercial insurances.

Intake Procedure

New patient registration/information form. Income documentation required to use sliding fee scale.

Languages

English, Spanish. Other languages available through an online interpreter service.
