Pregnant, Breastfeeding, Postpartum Women and Infants 0-5 yrs. Must be a Kansas Resident, Meet income guidelines and have a nutritional risk.

Intake Procedure
Provide proof of address, income, identity and immunization record.

English, Spanish, offer Language Line assistance.
