Intake Procedure
Call 620-326-9996. Medicaid cardholders needing transportation for medical appointments need to schedule their appointments through their insurance. The insurance company will pass along the needed trip to one of their providers. Request Futures Unlimited if WALT is preferred.

Mainly Wellington, KS. Out-of-county trips are available with advance notice for $1/mile if they occur M-F between the hours of 9:30 am and 3 pm and can be scheduled to take place after the 15th of each month.
