Description:
Community-Based Services (CBS) includes an array of intensive services provided in our community with the goal of maintaining children in their own homes. A child or teen must live with a seriously emotionally disturbed (SED) to receive CBS. Similarly, adults who have significant functional impairments resulting from an identified mental health or substance abuse diagnosis can receive Community Support Services (CSS). Services can be provided in the home, at school or in the community. FSGC!s trained staff works closely with individuals and families to identify each person!s unique needs and determine what services will be most effective. Other agencies and resources may be and often are included in developing the person!s treatment plan. The treatment plan is developed with a strengths-based model that focuses on what the individual and family do well.
