Eligibility
Serve families with children birth to 5 years old. Income, risk factors and migrant status (must be eligible for service through one of our grant funders to attend).
Fees
None.
Intake Procedure
Call for appointment to fill out application. Bring proof of income for last 12 months, child birth certificate, child social security card, medical card, current physical, hemoglobin, blood lead results, and immunization record.
