Federally qualified health center. Mission is to provide care to the community regardless of their ability to pay. Primary care, immunizations, well woman, well man and well child exams. Basic lab work (blood and urinalysis) and digital x-ray, referral dental services and behavioral health specialist services available. Have on D.O. and M.D. on site as well as two nurse practitioners.

8 am - 5 pm Mon - Fri.

For insured patients the fee is a regular office visit copay. For those without insurance the cost is dependent on your annual income and number of household.

Intake Procedure

To establish the Community Health Center as your primary care physician we do have a packet of new patient forms that our staff are happy to assist patients with. These forms are also available online at www.cowleyhealthcenter.org. In order to qualify for the sliding discount fee for those without insurance please bring two forms of identification and proof of income. If you do not have a source of income we have a form that you are able to pick up from our office or online.
