Exclusive Provider for Program for All-inclusive Care for the Elderly (PACE) in Sedgwick County. Specializes in comprehensive healthcare for older adults living with chronic diseases or with disabilities. Services include (but are not limited to):
!¢ Adult Day Center with Social Activities, Meals, & Outings
!¢ Transportation to our Center and Medical Appointments
!¢ Nursing & Physician Medical Care
!¢ All Prescription and Prescribed Over-the Counter Medications
!¢ Physical and Occupational Therapy
!¢ Specialty Care including Dental, Vision, Hearing and Foot Care
!¢ Social Services Supportive Case Management
!¢ Medical and Caregiver Respite
Hours:
8 am - 4:30 pm, Mon - Fri. (RN on call 24/7).
